• Assist patients with navigating insurance, benefits, and access to services.
• Maintain accurate documentation and case records in compliance with standards.
• Educate patients and families about treatment options, follow-up care, and resources.
• Partner with clinical and administrative teams to ensure seamless, patient-centered care.

What You Bring

• Background in social work, psychology, or a related healthcare field.
• Hands-on experience in patient-facing or case management roles.
• Strong communication and interpersonal skills, able to build trust quickly.
• Organized, detail-driven, and comfortable managing multiple cases.
• Tech-savvy, with proficiency in Google Suite, EMR, and CRM systems.
